Mailing Name	Text	Mailing name of the giver

State	Text	State in which the giver resides
Zip Code	Alphanumeric	Zipcode in which the giver resides
Phone Number	Number	Phone number of giver
Email Address	alphanumeric	Email address of giver
MapQuest	Button	Link that displays address location in MapQuest.
Date	Date	Date the gift was given
Amount	Decimal	Amount of the gift given
Designated For	Text	Area to enter for what the gift was designated.
Date Acknowledgement Sent	Date	The date that an acknowledgment notice was sent to the giver.
